Aged care assessment

Aged Care

Looking after older patients takes time, patience, and a doctor who gets the whole picture. Our GPs do 75+ health assessments, dementia evaluations, and ongoing chronic disease care. We also visit local aged care facilities to see residents in place. Dr Aye Mon has a particular interest in geriatrics.

What a 75+ health assessment covers

The annual 75+ assessment is fully covered by Medicare. Your doctor will look at your physical health and mobility, review all your current medications, check falls risk and home safety, screen memory and thinking, look at nutrition and weight, check hearing and vision, and ask about how you're going emotionally and socially. The point is to catch any new problems early and keep your overall plan up to date.

Support for carers

Looking after an older family member is hard work, physically and emotionally. We can give carers their own health checks, support mental health, point you toward respite options and carer support services, and help you understand the My Aged Care process. You don't have to manage it all alone.

Allied health referrals are coordinated by your GP during your consultation. Most allied health services are provided by external practitioners in the local area. Your doctor will arrange the appropriate referral and provide you with the necessary documentation. Some allied health services may attract a Medicare rebate when referred under a care plan.

Frequently Asked Questions

What is a 75+ health assessment?

An annual health check for Australians aged 75 and over. It's a thorough look at physical health, medications, falls risk, memory, nutrition, continence, mental wellbeing, and anything else that's come up. Medicare fully covers it.

Do you visit aged care facilities?

Yes. Our doctors regularly visit local aged care facilities for routine assessments, medication reviews, chronic disease management, and urgent consults when residents need them.

What's involved in a dementia assessment?

Cognitive screening tests, a behavioural review, a check of medications, a physical examination, and a chat with family if they're involved in care. The GP may refer to a geriatrician or neurologist for more detailed assessment.

What support is there for carers?

Caring for an older parent or partner is exhausting. We can do carer health checks, mental health support, help you find respite, and help you navigate My Aged Care. You don't have to figure it all out yourself.
